This form is for professionals and families to make contact with the Bristol Family Hubs service and request support.
Bristol Family Hubs provide support and information for families with children up to age 19, or young people up to age 25 with special educational needs and disabilities (SEND).
You must have the consent of the parent or carer in order to make this referral. The family will have had to consent to information being shared with Bristol Family Hubs and have agreed to be contacted by a member of the Family Hubs team.
Families will become a member of Bristol Family Hubs to receive support. You can support the family to sign up to Bristol Family Hubs. If this is not possible to do in advance, the family can do this on their visit to us.
Complete the referral in full, and with as much information as possible, so that we can ensure that your referral enquiry can be dealt with appropriately.
