Refer your child for Occupational Therapy

Before you start

As the child’s main carer, you're responsible for carrying out safety measures for your own children.

Make sure you:

read the information in the Safety Booklet
put in place all the appropriate recommendations before making this request

Do not make an Occupational Therapy referral if you have a safety concern around inappropriate housing and waiting lists for rehousing. Our assessment does not influence priority decisions made by HomeChoice. Ask a professional who knows your child well to send supporting evidence to rehousing.evidence@bristol.gov.uk.
