Bristol Educational Psychology Service uses psychology to support happy, effective learning and wellbeing.
Fill in this form to request the involvement of an educational psychologist (EP) for a child or young person in your setting who is under the age of 16.
Before you fill in the form you must:
When you submit this form you will receive a link to a separate version of this form that parents or carers need to complete to agree to this EP involvement going ahead. You will need to email this link to the parents or carers of the child or young person.
Parents or carers need to fill in the form within two weeks of you submitting this setting form in order for EP involvement to go ahead.
All parents or carers with parental responsibility should be included in agreeing this EP involvement. Send the link for the parent or carer form to the verified email addresses of all those with parental responsibility for the child or young person. However we only need to receive one parent or carer form within two weeks for the EP involvement to go ahead.
If the parent or carer has difficulties reading or speaking English you should support them to fill in the form emailed to them. There is the option to use Google Translate to support with completing the form.
We'll only start our involvement when we've got both forms. If we do not receive the parent or carer form within two weeks of you submitting this form, we'll delete this form and no further involvement will be possible at this stage.
If we do not receive the form within two weeks the EP expecting to be involved in this work should contact you to let you know that the parent or carer form has not arrived. You can also email tradingwithschools@bristol.gov.uk two weeks after submitting this form to check that the parent or carer form has been received.
If the form hasn’t been received you can submit another form. You'll need to agree with the parent or carer, and support them if necessary, to fill in their version within two weeks of your submission.
If parents or carers do not have a functioning email address, ask your EP for a hard copy of the request for EP involvement form or email tradingwithschools@bristol.gov.uk.
