Elective home education notification

Fill in this form so that school staff can share relevant information with the Education Welfare Service. You'll need to upload a copy of the parent or carer's written request to remove their child from roll and the reason for home education.

You can use this form to notify us of a child and up to three siblings.  If more than four children you'll need to fill in another form for the additional children.

Use this form when:

• the parent has told the school that they are assuming all responsibility for the child's learning
• the parent has contacted your school to request that the child is taken off roll

or

• you have reason to believe that the child is now being electively home educated by their parent
• the child has not started at your school and your reasonable enquiries show that they are being home educated
