Get a disabled persons bus pass

To apply for a disabled persons' bus pass you must:

• live in the Bristol City Council area, you can check which council area you're in
• have an eligible disability 
  
  

To complete this form, you'll need to upload:

• proof of your address such as a household bill, driving licence or bank statement
• a passport photo, digital or scanned photo on your computer: see rules for passport photos on GOV.UK
• evidence of your disability
  
  

Evidence of your disability

You can either:

• upload one of the following:
  • Proof of Attendance Allowance
  • Proof of the higher rate of mobility for Disability Living Allowance
  • DWP letter awarding Personal independence Payment (PIP) with a score of at least 8 points for “Moving around”
  • DWP letter awarding Personal independence Payment (PIP) with a score of at least 8 points for “Communicating verbally"
• get your doctor to complete a Medical Evidence Form that says you're unable to travel alone on a bus for medical reasons. You can either scan and email the form, or post it to us. You'll find details at the end of the form.
